On the one hand it's irritating as hell to have the server come around multiple times with those goddam pincers and play 20 questions with everyone around the table; huge waste of time, and frankly kind of embarrassing: "Holy shit! Lookit Norbert go. No wonder he weighs 400 pounds! Why not tie the basket around his blimp of a head?!"
On the other hand, putting out a basket on every table, only to throw it out after all the cry-baby gluten and carb haters refuse to touch it, seems like a greater waste of time, and some truly wonderful breads.

Greek salad. At least at anyplace other than an authentic Greek restaurant. Diners are not Greek restaurants.
Salade Nicoise. Unless it's like the one I can get anywhere in France, with, (gasp!) canned Italian tuna, french green beans, nicoise olives, and fresh anchovies, forget it. Half-heartedly grilled tuna steak should be reserved for that Greek diner's Friday night fish menu.
