Where to eat: Barcelona and Madrid

I'm going on a vacation next week to Barcelona and Madrid (with a day trip to Toledo). If anyone has any recomendations for places to eat--I'm looking for lunch, dinner, tapas (or the catalan version), bars, etc--any suggestion is appreciated!
