Using a Pizza Stone

I have had a pizza stone which I had purchased about ten years ago and used a few times, then stored in the garage and forgot about. I just pulled it out and want to use it tonight for a "take and bake" type pizza we bought at Sams. I know I should put the stone in a cold oven, then preheat the stone & oven together, but I am not sure how long to let it heat. Also, it seems like the stone was supposed to be rubbed first with cornmeal or something to keep the pizza from sticking--is that right? I am hoping for a crispy crust. Any tips on using this relic would be appreciated!
