Underground Restaurant

I'm looking for advice from anyone that runs an underground restaurant. How do you protect yourself from a lawsuit say if an individual were to get sick or have an allergic reaction after eating your food; or even tripping and possibly injuring themselves? Do you require your guests to sign a waiver or release form? If so, how is it worded and what would I need to do to I make it a legal document?
