I've always wanted to eat at El Bulli, which will require much planning but I'm hoping to make it there by next year. In the meantime, I wouldn't mind flying to Chicago for a meal at Alinea.

I always wanted to eat at Emeril's, and I finally got my wish. It was at the Emeril's in Orlando, and it was worth the wait. The appetizer was a wild mushroom dish, and the entree' was andouille encrusted redfish. Best meal I've ever eaten, bar none.

Heston Blumenthal's Fat Duck, Hot Doug's in Chicago and, if I'm being really honest, a Sonic Drive-In—I've never been but their commercials always kill me, it's cruel to run them in NYC when there isn't one near us. I finally got to try In-N-Out last year and happily, it was everything I hoped for and more.

Sanford in Milwaukee, WI and L'Etoile in Madison, WI. Two restaurants that make Gourmet's top 50 restaurant list (as well as many others) are right here in Wisconsin and I have yet to visit either.
In NY I would like to visit Le Bernardin and Masa and I would like to check out Shake Shack to see what all the fuss is about. Of course I would love to visit Alinea and the French Laundry.
As for casual eats, I echo an earlier comment about Hot Dougs in Chicago.

El Rey de Cabrito in Monterrey, Mexico. I assume Robb Walsh has been there but I haven' read that book yet, but I guess I should because it could be not so great anymore I suppose. Baby goat, fresh corn tortillas, a cold 32 oz Carta Blanca with lime and a small glass...missed out on a chance about 10 years ago and have thought about it often since, especially right now. Oh, and also Thailand.

per se, French Laundry (I'm a fan of Thomas Keller, in case you don't read my blog too often)...
Other places in NYC (or near it): Masa, Blue Hill at Stone Barns, Babbo, WD-50, and the highly anticipated P*Ong (a dessert bar owned by Pichet Ong).
Outside of NY: El Bulli, macarons from Pierre Herme in Paris, and a ton of other places that I can't name right now and if I ever have the money to do so.

I'm more of a "macro" guy. I don't know specific restaurants all around the world but here are certain countries I'd love to eat in...
Japan, Lebanon, France, Italy, Mexico.
And in the U.S., I'd like to give San Francisco another shot, preferably with more money, and while I absolutely love the burritos there, I'd love to eat somewhere else than burritos.
My current favorites are Vietnam and Thailand, and my current home, New York, a city with a seemingly infinite number of restaurants.

Sara, if it is of any help, I was fortunate to try one of Mauro Uliassi's dishes at a tasting event here in New York. It was gnocchi with duck ragu and, more memorable, pecorino di fossa. It is a fairly particular cheese that I have tasted only in Italy or when friends and family bring it back. The only other place I have seen it is at Babbo. Really, it was so simple and all about the cheese.

When you say "always," well, always is a long time. This means only older, established restaurants it seems to me. Most of the places on the list are more for historical thrills, less for culinary thrills.
NYC: Tavern on the Green, The Rainbow Room, Carnegie Deli and someplace with one of those "perfect" NYC pizzas we always hear about.
SF: Washington Square Bar and Grill, Tadich Grill, Fior D'Italia, and Gold Spike. Across the bay: Chez Pannise where the seasonal, local, and sustainable modern style of cooking (arguably) began.
