Repast in Atlanta, Georgia. It was an experience like nothing I've ever had. I felt like the restaurant was revolving around my chair, and I'd give anything to be able to do it all over again.

There is a fun chef's table (really a counter) at Resto in NYC. It's probably the most casual restaurant you will ever find a chef's table in. And the food is crazy good, though the chef loves fat more than life itself. My kind of cook.
